Review the investigation and name the one omitted fact that separated the final explanation from alternatives. Tie it to the branch it clarified, such as role, sequence, input, timing, or expected outcome.
Compare clarification paths
Look at how the detail was eventually obtained and how much back-and-forth or uncertainty it removed. Distinguish a useful prompt from information that was merely available after the fact and not necessary for the decision.
Adopt one safer prompt
Update one intake question, support handoff, or internal checklist with a clear fallback when the detail cannot be shared. Assign an owner and define a future observation that tests faster bounded triage.
What to carry forward
The learning output names the decisive reproduction detail, why it mattered, and one privacy-conscious way to request it earlier. Stop when the change has an owner and test; do not turn a single difficult case into a mandatory evidence burden.
