Set a risk order
Queue confirmed live effects and shared routing or credential gaps first, then isolated test failures and missing labels. Add a fixture, review date, and escalation trigger. Keep impact provisional where source identity or resource outcome cannot be verified.

Return an environment-separation queue with possible effect, scope, containment, owner, and proof. Escalate confirmed live resource changes or shared credential use. Keep rank provisional when event origin, endpoint configuration, or downstream state remains unknown. Assign one explicit owner.
