Establish what is happening
Compare each cycle
List the requested outcome, closure reason, evidence, reopened trigger, and affected context for every cycle. Preserve differences in release, role, input, and scope so separate problems are not collapsed into recurrence.
Name the unresolved condition
Identify what remained false or unknown at each closure: behavior, acceptance evidence, dependency, communication, or customer outcome. Treat a reopened comment as evidence to classify, not as the complete cause.
Set a bounded state
Keep open, split, investigate, verify, or close provisionally with an explicit condition and owner. Define the smallest check that distinguishes a recurring failure from a new request. Set a condition that distinguishes unfinished original scope from a genuinely new request.
What to carry forward
The output is a recurrence record that separates cycles, unresolved conditions, and scope changes. Stop triage when the next state and closure condition are clear; if causes remain mixed, split the work or keep it open with named uncertainty.
