Learn how decision context gets lost from work items

Lost context is a documentation symptom with real coordination cost. Learning should compare the original decision path with the reconstruction effort, identify the first durable link that failed, and improve one record practice without requiring exhaustive meeting notes. The improvement should preserve enough rationale for a new owner to act without recreating the meeting.

Keep the lesson for the next incident

  1. Trace context decay

    Review where rationale lived, which links survived, and when the work changed owner or scope. Identify the first transition where a new collaborator could no longer recover the decision from the record.

  2. Compare recovery cost

    Note repeated questions, delayed decisions, duplicated work, reversal risk, and confidence gained from reconstruction. Keep the lesson specific to the item’s actual dependency and decision boundary. Compare which durable link would have reduced the reconstruction effort most.

  3. Strengthen one context link

    Adopt one adjustment such as a decision record link, rejected-alternative field, assumption review, handoff summary, or scope-change prompt. Assign an owner and future item to test whether context remains recoverable.

What to carry forward

The learning output connects context loss to its first failed link, recovery consequence, and one testable record improvement. Stop when the change has an owner and boundary; preserve uncertainty rather than filling future records with unsupported narrative.
