Build a useful investigation brief
Build a handoff lineage
Collect task brief, evidence index, analysis notes, calculations, conclusion, open questions, owner decisions, and summary revisions. Map each summary statement to its source or mark it unsupported. Record when scope, caveats, or status disappeared during editing.
Measure receiver friction
Give the summary to a reader who did not perform the work and ask for the decision, scope, evidence, uncertainty, and next action. Compare answers with the underlying brief. Record every reconstruction question and the missing field that caused it.
Test a smallest amendment
Add one missing context item at a time, such as artifact revision, conclusion, caveat, owner, or next action, then repeat the reader check. Identify which addition restores a correct continuation. Keep broader template changes separate from the incident-specific repair.
What to carry forward
Produce a handoff investigation with source lineage, omitted fields, receiver questions, amendment tests, and remaining work. Recommend a concrete summary repair or template change. Do not blame brevity when the evidence shows a missing artifact, conflicting scope, or unfinished decision.
