Save the handoff case
Store request, agent state, trigger, transfer text, packet, recipient interpretation, user notice, and outcome together. Include a case where an early transfer prevented risky continuation and a case where escalation happened without enough context.
Improve evaluation practice
Add trigger timing, context completeness, ownership, next-action, and recipient-readiness checks to the evaluation plan. Require a non-handoff control. Explain how the new checks address the exact missing field or late escalation observed.
Watch for transfer debt
Set a signal such as repeated requests for the same history, handoffs without owners, user replies asking what happens next, or escalations after state changes. Assign an owner to sample packets and revisit the checks when the signal returns.
What to carry forward
The learning record should connect the observed transfer gap to the revised handoff checks and recurrence signal. State which packet or trigger rule changed. Keep the lesson tied to the tested recipient path rather than prescribing one universal escalation style.
