Watch for commitment drift
Set a signal such as green status without source links, milestones used as evidence, or solution scope expanding after unresolved questions. Assign an owner to sample future readiness reviews and revisit the practice when the signal returns.

The learning record should connect uncertainty to the readiness practice and recurrence signal. State how the next decision will preserve a reversible boundary. Keep the lesson tied to evidence and context rather than converting one review into a universal gate.
