Trace which report, workflow detail, or support signal exposed the issue and which evidence was absent at first. Note whether the cohort was hard to observe because of channel coverage, permissions, or a missing product signal.
Compare scope and outcome
Look at later reports and verification results to see whether the issue remained narrow, expanded, or split into different conditions. Preserve the original reach estimate and distinguish new evidence from a changed definition of the cohort.
Improve severe-case handling
Choose one adjustment such as an exposure question, a cohort field, an escalation threshold, or a follow-up check. Assign an owner and define the observation that will show whether the adjustment catches severe narrow cases earlier.
What to carry forward
The learning record links the severe outcome to its discovery path, later scope evidence, and one concrete review improvement. Stop when the adjustment has a measurable follow-up and does not overclaim prevalence; preserve the small cohort as a meaningful boundary condition.
