Keep the lesson for the next incident
Keep the role evidence
Record event, role, ownership, target state, action, visibility, label, destination, response, and fallback. State the safe outcome for each role. This prevents later UI changes from treating a successful owner path as universal.
Compare later roles
Review another role, changed state, and direct notification path after the change. Look for a control that remains misleading or a read-only person who loses context. Preserve a clear owner and observer control.
Refine one scope check
Require future reviews to pair role and state, inspect action visibility and response, and test a read-only destination. Assign ownership and revisit date. Define the observation that would show the check reduced wrong affordances without removing useful review access.
What to carry forward
The learning record connects one role mismatch to later evidence and a reusable action-scope review. Stop when another reviewer can repeat the role-and-state review. Carry forward uncertainty where roles or target state cannot be reproduced directly.

Keep the decision with the work.
Use a Work Item in Aglet to record the problem, the evidence you have, and the next decision. Add an owner and priority, then keep updates in the discussion so the next person can follow the reasoning.
