Locate the loss
Compare episodes across repository, build, provider, ingestion, and presentation boundaries. Identify whether the same field is absent, transformed, delayed, or overwritten. Preserve cases where the release truly had no immutable identity.
Record the fallback
Document the most trustworthy secondary anchor available when a commit is missing, such as an immutable artifact or runtime marker. State exactly what decisions that anchor supports and which it cannot support.
Define recurrence recognition
Set a rule for reviewing future releases with missing identity and name the owner of the context check. Include the first downstream decision that should pause when identity cannot be confirmed.
What to carry forward
The lesson should produce one identity-capture or review change and a recurrence rule for missing or conflicting references. Do not claim the gap is eliminated until later release records retain enough immutable context for independent verification. A useful follow-up can require an immutable artifact or runtime marker at release review while preserving a documented exception for releases that truly lack source identity.
